Cold, Tired, Anxious, and Losing Hair? That's Not Four Problems. That's One.

Cold hands, thinning hair, a stalled gut, and anxiety that showed up out of nowhere aren't four separate problems. They're one: a drop in how much thyroid hormone is actually reaching your cells, showing up as five different symptoms depending on which tissue is being asked to run on less energy than it needs.
By the time most people land in my clinic with this pattern, they've usually already collected a small folder of specialists trying to solve each piece separately. A dermatologist for the hair coming out in the shower. A GP for the constipation that laxatives only half-fix. Sometimes a psychologist for the anxiety that showed up out of nowhere in their thirties or forties. Maybe a cardiologist, at some point, because their resting heart rate looked odd and someone got worried. Each of these was treated as its own small mystery, worked up, tested, and managed in isolation. Nobody sat down and asked whether a person who is cold all the time, losing hair, constipated, anxious, and exhausted might be dealing with one problem wearing five different costumes.
They usually are. Once you see the connecting thread, it's hard to unsee it in almost every thyroid case that walks through the door.
Why Are Cold Hands, Hair Loss, a Slow Gut, and Anxiety All Connected?
The connecting thread is thyroid hormone, specifically how much of it is actually reaching your cells and getting used. It's one of the primary signals that tells almost every tissue in your body how much energy to produce, and when that signal drops, whether from Hashimoto's, subclinical hypothyroidism, or a thyroid that's technically "in range" but running low for that particular person, the shortage shows up wherever a tissue depends on that energy to do its job properly.
Think of it less as a hormone specific to one gland and more like a dial that sits inside nearly every cell you have, turning metabolic output up or down depending on how much signal is reaching it. Different tissues just show the strain differently, depending on what job they're being asked to do with less fuel than they need. That's why the symptom list looks so scattered on paper, hair, gut, mood, temperature, when it's actually one signal, read five different ways by five different systems, each translating "not enough energy available" into whatever symptom that particular tissue produces under strain.
The Fatigue and Weight Gain Nobody Can Explain
Fatigue and weight gain happen because thyroid hormone, particularly the active form T3, switches on the machinery inside your cells that builds and runs your mitochondria, the actual furnace that keeps your metabolism running. It does this by binding to receptors that directly activate the genes responsible for building and running your mitochondria, which means T3 isn't just fuelling your metabolism, it's instructing your cells to build more of the machinery that produces energy in the first place.
When T3 signalling drops, that instruction doesn't go out, or goes out at a reduced volume. Basal metabolic rate falls, cellular energy output falls with it, and the felt experience is exhaustion that a full night's sleep doesn't touch and a body that seems to gain weight on the same food that used to maintain it. This is one of the more disorienting parts of thyroid dysfunction for people to sit with, because it looks, from the outside, like a willpower problem. It isn't. It's a furnace running on a smaller flame than it used to.
Why You're Cold When Nobody Else Is
The cold intolerance comes from a more specific version of the same story. Your body has a dedicated tissue, brown adipose tissue, whose entire job is generating heat by burning fuel inefficiently on purpose, a kind of built-in radiator most people never think about until it stops working properly. It does this through a protein called UCP1, and the signal that switches UCP1 on is, again, thyroid hormone acting directly on those cells.
Human studies have shown that when hypothyroidism is treated and thyroid hormone is restored, cold-induced heat production in this tissue comes back online, which is about as clean a demonstration as you'll get that the "why am I always cold" complaint isn't in your head, and isn't really about the weather either. It's a heat-generating system that's been told to power down because there isn't enough signal reaching it to justify the energy cost of running it. Cold hands and feet, in this context, aren't a circulation quirk. They're a resourcing decision your body is making on your behalf.
The Hair Loss That Shows Up Months Later
The hair loss follows a similar logic, just on a slower timeline, which is part of why it confuses people so much. Hair follicles cycle between a growth phase and a resting phase, and normally only around one in ten follicles is resting at any given time. Thyroid hormone receptors sit directly inside hair follicle cells, and when thyroid hormone drops, that balance shifts hard, sometimes pushing thirty to seventy percent of follicles into the resting phase simultaneously.
Hair doesn't fall out the moment that shift happens. Resting follicles hold onto their strands for a while before releasing them, which means the shedding people notice in the shower is usually a two to three month echo of whatever was happening to their thyroid function back then, not a reflection of what's happening now. That lag is exactly why so many people, and often their doctors, don't connect the two. It's not really a hair problem in the sense most hair loss treatments are built to address. It's a follicle that's been told to stop investing energy in growth because energy is being rationed elsewhere in the body.
The Gut Slowdown That Isn't About Fibre
The gut tells the same story again. Thyroid hormone helps drive the smooth muscle contractions that move food through your intestines, and when that signal weakens, transit time slows down across the board. This isn't only true once someone crosses into a clinical hypothyroid diagnosis, either: a large study of people whose thyroid function was still technically within the normal range found that lower thyroid hormone levels were associated with more constipation, in a dose-dependent way, meaning the gut appears to respond to the gradient, not just to the label of "hypothyroid" or "not."
Constipation, in this context, isn't a separate digestive issue to solve with more fibre or a different laxative. Fibre can help around the edges, and I'm not suggesting people stop using what works for them, but it's working on the downstream symptom rather than the upstream cause. The muscle wall of the gut runs on the same energy signal as everything else on this list, and when that signal drops, motility drops with it, regardless of how much soluble fibre is sitting in the intestine waiting to be moved along.
When Anxiety Isn't "Just Anxiety"
Anxiety is the one people find hardest to connect to their thyroid, because it feels psychological rather than physical, but the biological link is well documented. Thyroid hormone influences the neurotransmitter systems that regulate mood and calm, including GABA, serotonin, and noradrenaline, all three of which depend on adequate thyroid signalling to function the way they're meant to. A large review covering more than 36,000 people found that those with hypothyroidism were over twice as likely to also have an anxiety disorder.
It's worth being honest about what that kind of evidence can and can't tell us. This is a strong, consistent association, not proof that low thyroid hormone causes anxiety in every case, and the researchers themselves frame it as an open question of causation versus co-occurrence. Stress, sleep disruption, and the exhaustion of dealing with four other unexplained symptoms all plausibly contribute too, and untangling how much of each factor is doing the work is difficult from the outside. But the mechanism is real, the pattern is common in clinic, and it explains why so many people are handed an anxiety diagnosis in the exact years their thyroid symptoms were building underneath it.
Five Symptoms, One Bottleneck
None of these five threads exist in isolation from each other. They're what it looks like when a system-wide energy signal drops and every tissue that depends on it responds according to its own particular job: thermoregulation, hair growth, gut motility, mood regulation, general metabolism. Each tissue is doing exactly what it's supposed to do with the resources it's been given. The problem was never located in the hair, the gut, or the nervous system individually. It's upstream of all of them.
That's also why treating each symptom on its own so often falls flat. Fibre supplements for constipation, iron infusions for fatigue, SSRIs for anxiety, and topical treatments for hair loss can all help around the edges, and there's nothing wrong with using them while the underlying picture gets worked out. But none of them address the actual bottleneck generating all four symptoms in the first place, which is why people so often end up on four separate treatments, feeling somewhat better in isolated pockets, while the thing underneath all of it goes unaddressed.
Why This Gets Missed on Standard Testing

A single TSH result sitting inside a wide reference range doesn't rule out this whole pattern. It usually just means the one number being checked wasn't sensitive enough to catch the part of the system that's actually under strain.

It's usually one system asking for more than it currently has the capacity to give, and every symptom on that list is a clue pointing back to the same place.
